That's the kind of data I was looking to generate. We didn't have that panel long enough (or used enough) for it to be a useful surrogate. The antibiogram is enough to show the lack of CRO-R isolates.

You'd have to look at all the people empirically treated for meningitis to have a useful denominator, so they would not have that info.

We tried to use ceftriaxone 2gm q12h as the identifier but found that most of it was for either enterococcal IE or was an error. I expected more empiric CNS.

I did a similar search, I just don't trust my own searching when trying to find where a practice originates. Gave a presentation at IDWeek about IV vs PO abx in BSIs - finding the data pushing that practice was torture and took me through the decades.

I tried a local study to look at this and couldn't create a numerator. So the number of people to get vanc before a benefit was infinite.

Do we even know if ceftriaxone-R meningitis has higher morbidity/mortality than CRO-S if CRO if CRO is given without vancomycin? I'm going to guess we do not.

I think those who hold onto clindamycin etc are just not up-to-date. You can't blame them- they are in primary care, and atorvastatin works the same today as 2/
