Drs office, gives me a 25 page work sheet to do before becoming a patient

Then sends me a 15 page ONLINE work sheet to do

Makes me fill out 7 pages at the office at every visit

Then proceeds to force me to fill out a 16 page online worksheet BEFORE every visit
