Administration to phase out NIH support of HIV clinical guidelines The National Institutes of Health will phase out its support of the clinical practice guidelines that advise doctors how to diagnose and treat HIV.

Phasing out NIH’s HIV clinical guidelines is flat-out immoral and unethical.

These guidelines drive early diagnosis, ART initiation, and viral suppression—82% linked to care within 30 days, 69% virally suppressed by 6 months.

Removing them puts millions of lives at risk.

Non-pharmaceutical interventions and covid-19 burden in the United States: retrospective, observational cohort study To evaluate the adoption and discontinuation of four broadly used non-pharmaceutical interventions on shifts in the covid-19 burden among US states. I just listened to #TheDaily episode on "On COVID's Wake" with Frances Lee & Stephen Macedo where they make a categorical statement that non-pharmaceutical interventions didn't work during the pandemic. The evidence does not support this categorical claim. 1/

The Love Tank calls for pharma diversity commitment — The Love Tank CIC In a letter to the Chief Executives of GSK and ViiV Health Care, The Love Tank today calls for a commitment for diversity from the companies.

“A UK registered company (GSK), rolling-over to a US administration that shows blatant disregard for human rights, sends a chilling signal to partners & sets a dangerous precedent.

We join other NGOs & activists in expressing our gravest concern with GSK."

Finally I want to highlight the danger of associating drug use with HIV transmission. In the first mentioned JAIDS study, all of the cocaine using participants were undetectable. Furthermore I find the evidence of cocaine being the main driver for the significant difference in LHR to be weak at best

For example, do non-psychoactive non-illicit chemicals have similar interactions with HIV immunology which we will never discover?
Do 'socially accepted' stimulants (with similar structures) like those prescribed for ADHD and narcolepsy face the same studies?

To what extent can scientific research detach itself from societies stigma? I am curious if meth and cocaine were chosen for the cell studies due to the history of HIV incidence in drug users + the negative social capital associated with illicit drugs

Now, I don't want to stigmatise substance use in PLWH (or anyone for that matter) as it is ubiquitous, fun and abstinence does not work. Rather, I want to pose some questions that came up from this.

Cocaine enhances HIV-1 replication in CD4+ T cells by down-regulating MiR-125b - PubMed The main objective of this study was to examine effects of cocaine on HIV-1 replication in primary CD4+ T cells. Cocaine a commonly used drug among HIV-1 positive individuals serves as a cofactor for ...

But, in the paper, and its citations [45-47] there is proven interactions between cocaine and HIV, proven in vivo and in vitro.

In my training, drugs were always been linked to HIV/STIs through a social epidemiological lens. Chemsex, social status and the systemic marginalisation of illicit drug users tend to come to mind as the primary connection between the two.

So here is a thread of me yapping about this mornings readings and reflections.

I want to talk about HIV and Cocaine.

This morning I stumbled upon a publication linking recent cocaine use in PLWH to increased latent HIV reservoirs. This caught my eye and I was surprised by what the paper suggested and intrigued by its citations.
The paper is: doi.org/10.1101/2022...

Routine #HIV testing has arrived to the @nhsUHLG A&Es in #Liverpool!

Opt-out testing works, saves money, and it's finding people who would otherwise not receive a test, most often from communities who are being left behind in our progress on #HIV.

Private insurers labelling random people as living with HIV is appalling. Using HIV, Hepatitis C and other infections to fraud people for greater profits is absurd.

Exclusive | Insurers Pocketed $50 Billion From Medicare for Diseases No Doctor Treated Questionable diagnoses of HIV and other maladies triggered extra Medicare Advantage payments. “It’s anatomically impossible,” said one doctor about some cataract diagnoses.

Felt this WSJ article is a bit more topical following the recent news regarding private healthcare insurers.

The phrase "Insurer-driven diagnoses of HIV" is something I did not expect to see.
