What doesnโ€™t evolve, goes extinct!!

If you donโ€™t adapt new data, youโ€™ll never improve patient care. No study is perfect, and none ever will be. What matters is critical appraisal of available literature and adapting it to improve patient care ๐Ÿ™๐Ÿฝ

NOOOOOOOOOOOOOOOOOOOO!

โ€ขAny infection
โ€ขAny inflammation

โ€ขCancer

Remember, D-dimer is a very sensitive test but NOT a very specific test ๐Ÿ™Œ๐Ÿฝ

It's not just the โคต๏ธEPO that causes anemia in patients with CKD

CKD = inflammation = โคด๏ธHepcidin = โคต๏ธiron absorption = anemia

Blood loss during dialysis = iron deficiency anemia

CKD = โคด๏ธUremic toxins = โคต๏ธred cell production

Anemia in CKD ๐Ÿ‘‡๐Ÿฝ

We all know that CKD = โคต๏ธEPO

What's the mechanism?

The interstitial cells transdifferentiate into MYOFIBROBLASTS instead of EPO producing cells

โคต๏ธ EPO producing cells = โคต๏ธEPO = Anemia ๐Ÿ‘‡๐Ÿฝ

Clearance of Driver Mutations after Transplantation for Myelofibrosis | NEJM Allogeneic hematopoietic stem-cell transplantation is the only curative treatment for myelofibrosis. Driver mutations are the pathophysiological hallmark of the disease, but the role of mutation cl...

In a single-group, single-center study, clearance of driver mutations 30 days after bone marrow transplantation predicted long-term disease-free survival in patients with myelofibrosis. Read the full study results: nej.md/4fQNdKn

One cannot manage a patient with febrile neutropenia without having a sound knowledge of medicine!

You need to know medicine before you step into a hematology unit! Period.

Iโ€™ve cared for infants who stopped breathing w/ pertussis.

School aged kids who died of flu or COVID.

And many of my patients canโ€™t be vaccinated because theyโ€™re on chemotherapy or are immune compromised.

Vaccinations save lives of those who get them & those who cannot.

Itโ€™s that simple.
