However, I also think that there should be guardrails in place so that teams don't abuse this by drumming up fake injuries. So there has to be some sort of pre-agreed extent to which injuries are considered grave enough for the player to be replaced.

Here's my suggestion: Let the medical staff of both teams evaluate a player. If both teams' medical professionals come to a consensus that a player cannot continue, they are granted a replacement.
