Not sure what annoys me more:
• They clearly haven’t bothered updating this form in at least six years.
• There is no way to read this form so that “Fifth Disease” is fifth.

To their defense the kids should be vaccinated for first (measles), third (rubella) and we are not sure exactly what forth disease is/was.
