I used to have the problem that my USB Gamepads would randomly disconnect and reconnect. Tried similar things to you, even reinstalled windows. What fixed it was to get a USB hub with it's own power supply. Suspect a motherboard issue.

It's a stretch but maybe it's the same issue.
