The major failure is that following years of austerity there are minimal services to support families in difficulties that would enable children to safely remain within families.

Overall, in my experience (as a recently retired expert witness in Family Court proceedings) most Local Authorities in Cymru take the protection of children very seriously (cf some LAs over the border whose thresholds leave children living in appalling conditions - personal view).
