It very much does involve figuring out a way to tax unrealized gains / stock values. Say for every $500M in a stock portfolio, you are taxed 1%, split up over 4 quarters. Each quarter you're taxed on the average value for that quarter. Some normalizing done at year end.

1) create an equitable physician payscale (start with the current Medicare one but better adjust for regional cost differences

Do the taxed assets include stock valuations? So many of the ultra rich have the majority of their wealth in stocks, giving them access to money, security, power, and influence, but we don't have a good way to tax that wealth until it gets used. We need to tax those assets as well.
