Many consultants and PGTBs support LTFT in theoryโbut despite a lack of evidence for any significant negative impact, some trainees fear negative perceptions from others about competency & career progression. 7๏ธโฃ

Trainees complete most mandatory training requirements in their own time. Many doctors with caring responsibilities have considered leaving their specialty due to a lack of options. LTFT could help reduce attrition and burnout. 6๏ธโฃ

๐ Key finding 2: Postgraduate training bodies (PGTBs) could do more to support LTFT training.
Many doctors donโt even know LTFT is an option, and existing processes are complex and restrictive. Clearer guidance, better admin support, and active promotion of LTFT are needed. 5๏ธโฃ
Quote from training administrator: "[LTFT should be] planned and in a managed way rather than, I suppose, in an ad-hoc way and sort of expecting it to work without actually putting the plan in place โฆ saying, โOK, itโs flexible training and there is the option, but thereโs no resources or โฆ backup behind it.โ"
๐ Key finding 1: Current training structures are inflexible.
Admin challenges are real. Payroll, compliance, & rostering all become more complex with LTFT. A few trainees currently slot-share at 50% WTE, but many would prefer higher WTEs of 60โ80% for a number of reasons. 4๏ธโฃ
Our group at the Irish College of General Practitioners, College of Anaesthesiologists of Ireland & Royal College of Surgeons in Ireland conducted interviews across multiple specialties & backgrounds including doctors, administrators & educators. Thematic analysis was used to identify core themes. 3๏ธโฃ
