One thing about me is I will always listen to Mr. Brightside when it comes on. Sometimes I even listen to it on purpose. Happy Friday.

This was one of the best discussions I've seen at SCCM--thanks to @pccmdoc.bsky.social for organizing and to all the panelists for their contributions

Absolutely. This fits systematically with their overarching goals - the whole government has an enemies list. Not great that so far that includes the FBI, the NIH and international aid. Still waiting on those cheaper eggs.

As chilling a meme as I have seen. But I agree.

Trump hits NIH with β€˜devastating’ freezes on meetings, travel, communications, and hiring Researchers facing β€œa lot of uncertainty, fear, and panic”

Hopefully this is only a short freeze but it can still have negative consequences, contrary to Trump claims of making America healthy. The bigger long term concern is over how the Trump administration will hinder scientific research beyond this freeze.

I think we have to accept that our country wanted this. They saw who he is and still wanted it. It makes me really sad and worried but denying electoral results is a huge problem and how you get Jan 6. We can't stoop to their level.

If you want democracy then you have to accept that sometimes the people are stupid. This is a trash take. Unfollowing.

Speaking from a lot of lived Januarys in the PICU for experience... Always helpful to see anecdote turned data.

Well put. I've seen a few cases of pertussis and one memorable epiglottitis, but remember my general clinic preceptor talking about doing 8-10 LPs per shift in the 70s. I agree that we seem to be heading back there, and many kids will suffer needlessly.

I try a new dressing recipe every year - haven't found the right one yet. I do love homemade green bean casserole though!

? From my last call. Sepsis is my jam, despite my current #pcicu life. We admitted a complex dorv kid with sepsis. EM resident on the case said the 'current recs' are to stop at 30/kg of fluid resuscitation if you don't have a PICU. Interesting interpretation of the FEAST trial (IYKYK). Thoughts?

CXR with over expanded lungs and squished heart CXR with lungs normally expanded (can see patchy infiltrate on right) and the heart is no longer squished

A tale of two ventilation strategies in status asthmaticus

Left: High respiratory rate, high tidal volume, high PEEP, targeting normal pCO2

Right: Low respiratory rate, low tidal volume, low PEEP, permissive hypercapnea

(I target pH 7.2 and put bicarb in the maintenance IVF if needed)

Hi Claire. Hope you are well. One of my favorite poems (although I'm pretty bad at poetry).

Absolutely agree. Very different from a lot of his other stuff but very connected too. And Randall Flagg shows up πŸ˜‰

As an editor I have the 'pleasure' of reading multiple clinical manuscripts each week.

Content is key, there is no getting around this.

But content needs to structured in an easily accessible way for the reader to engage...

Is this called a "skeetorial"? Anyway, here we go.

I'm on call tonight and alternately checking on a fresh BDG, learning how to sequence, and doomscrolling. But I turned 40 last month and if you had told me when I was young(er) I would be doing this still and love it this much I would have been happy for future me. Except for the doomscrolling.
