Yep, it's proven over and over again that it's better not to rely on the employees but make sure you build in a check against previously breached ones when users create a new password.

As this report highlights TU/e (University of Eindhoven) did a good job at IR, kudos to them for that.

Also yet another example that orgs get still hit through the weak spots and that this is often authentication (password/lack of MFA) related. 1/2
