Weβre proud of @pathwayuk.bsky.social's role in this research and excited to see the outcome. Huge thanks @brionyhudson.bsky.social, Dr Caroline Shulman, @inclusivepeolc.bsky.social for making this happen.
@mariecurieuk.bsky.social @uclpsychiatry.bsky.social
Our volunteers' experiences of accessing the healthcare system have been captured in several short films, an important resource for the IMPROVE programme (Intervention to optiMise Palliative caRe for peOple with liVed Experience of homelessness).
14.10.2025 10:11 β π 1 π 0 π¬ 1 π 0Last week our lived experience team contributed to NIHR funded research led by @pathwayuk.bsky.social Fellows, @brionyhudson.bsky.social, Dr Caroline Shulman and @inclusivepeolc.bsky.social. The realist evaluation of IMPROVE will optimise palliative care for people experiencing homelessness.
14.10.2025 10:11 β π 1 π 0 π¬ 1 π 0Here @pathwayuk.bsky.social, keeping the voices of people with personal experience of homelessness at the heart of our work is of highest priority.
This includes enabling people to share their experiences to influence health service development and policy, as well as contributing to research.
The case studies showcase services delivering the recommendations of the NICE Guidelines on Integrated Health and Social Care for People Experiencing Homelessness (NG214) π

We hope they inspire services tackling health inequalities in their local areas.
Along with @itsgroundswell.bsky.social and @homelesslink.bsky.social, we are publishing today a new case study resource showcasing best practice to address these health inequalities, highlighting how much effective collaboration across the health and homelessness sectors can achieve.
10.10.2025 08:52 β π 2 π 0 π¬ 1 π 1Today weβre marking World Homelessness Day.
The health inequalities associated with homelessness are so severe that lives are cut short by over 30 years, tragically many people dying from treatable conditions.
Today's figures confirm once again that homelessness is a health catastrophe.
We look forward to the Government making the right choices in the forthcoming homelessness strategy.
The deaths are all the more shocking given the vast majority are avoidable and the solutions to prevent them within our grasp π
Safe and secure housing, and, crucially, health services designed to meet the care needs of people facing homelessness, while supporting them out of homelessness.
The latest @ourmoh.bsky.social 'Dying Homeless' report lays bare the tragedy of 1,611 people dying in the UK while homeless in 2024.
We owe a debt of gratitude for MoH's work, going some way to make up for the invisibility of housing and homelessness in official mortality statistics and NHS data.
The achievements of the Pathway hospital teams highlight the critical role of the NHS in ending homelessness.
@pathwayuk.bsky.social is urging the Government to use the forthcoming Homelessness Strategy to implement key interventions in the healthcare system needed to tackle homelessness.
Alex Bax, @pathwayuk.bsky.social's CEO, gave a presentation on the hospital-based Pathway Partnership Programme teams who have a track record of ending and preventing homelessness.
These multidisciplinary, expert teams achieved a 62% reduction in patients returning to rough sleeping in 24/5.
Thank you @homelessnessimpact.bsky.social for todayβs Impact Forum on shifting homelessness from crisis to prevention.
With latest statistics revealing a 20% year on year rise in rough sleeping, it was excellent to join colleagues to explore evidence-based interventions which will drive change.

Without accurate data people facing homelessness are invisible in needs assessments and service planning, driving poor accountability.β―
βοΈ Making housing status a mandatory field in NHS data is a simple, low-cost, high-impact solution.
[2/3]
The average age of death for women and men facing homelessness is 43 and 45. They suffer from preventable and treatable conditions like asthma and heart disease.β―
βYet, there is still no systematic way to accurately record or identify patients facing homelessness in the NHS.
[1/3]
π Great to see @dannybealesmp.bsky.social raise the invisibility of homelessness within healthcare data with the Minister for Patient Safety, Womenβs Health and Mental Health during the Health Select Committeeβs Inquiry into Community Mental Health Services.
3. Recording patientsβ housing status in NHS data to inform effective planning & provision of services.
These interventions will tackle the severe health inequalities faced by people experiencing homelessness and help shift the dial on homelessness.

1. Scaling up specialist teams to support patients facing homelessness across the hospital network.
2. Investing in specialist intermediate care, to ensure people being discharged from hospital have somewhere safe to recover, with support to find long-term accommodation. π
Thank you @dannybealesmp.bsky.social for chairing today's excellent Parliamentary roundtable on the role of the NHS in tackling homelessness.
Convened by @pathwayuk.bsky.social, MPs heard about 3 evidence-based interventions which must be included in the Cross-Government Homelessness Strategy π
Our CEO, Alex Bax, was delighted to attend the launch of the APPG for Ending Homelessness' excellent new report today. With the imminent publication of the Government's Homelessness Strategy, it's great to see his evidence on the role of healthcare in preventing homelessness reflected in the report.

Having a show garden at #RHSChelsea provided an invaluable opportunity to raise awareness of:
β‘οΈ the catastrophic impact of homelessness on health
β‘οΈ the huge obstacles people experiencing homelessness face in getting the healthcare they desperately need
Co-production is at the heart of what we do. People with experience of homelessness advise across all our work.
This year Ross has been a key member of our #RHSChelsea show garden team - right from the start, through to the garden opening at its new home with @shekinahcharity.bsky.social.
We believe that everyone has the chance of a long and healthy life.
Thatβs why we're proud to join the @healthequals.bsky.social
coalition to support action on the building blocks of health and #MakeHealthEqual in the UK.
