They explained the impact of taking a whole-family approach to support for children with complex, chronic conditions by integrating the hospital with community settings.

We attended today’s new Academic Primary and Community Care Unit (APCCU) launch.

💡We shared our idea of a community challenge hackathon: bringing people together across sectors to look at the #SocialPrescribing evidence base and develop a framework for persuasive dissemination.
