On this anniversary I wrote a letter to Minister Ahmed, formally to exercise the powers under the Medicines and Medical Devices Act 2021 (Sch 1, para 3), to request information for the purposes of carrying out my core duties. 2/4

Friday 31 October 2025 marked the second anniversary of the Ministerial Advice I made to the DHSC on options for redress for those harmed by valproate and pelvic mesh. 1/4

Despite ongoing engagement, I still await a formal Government response. The recommendations are practical, proportionate, and deliverable, and would provide support to those who need it most. 3/4

Patient safety commissioner for Scotland recruited After two failed recruitment rounds a candidate for Scotland's first ever patient safety commissioner has been found.

Scotland has appointed Karen Titchener as its first Patient Safety Commissioner—a vital move to amplify patient voice and enhance safety.
