Implementation of a SMART-based infection control plan in an emergency intensive care unit: a prospective study | Scientific Reports Multidrug-resistant organism (MDRO) infections pose significant challenges in intensive care settings. This prospective study aimed to evaluate the impact of a Specific, Measurable, Achievable, Relevant, and Time-bound (SMART) infection control plan on MDRO prevention among healthcare workers in an emergency intensive care unit (ICU). Fifty healthcare workers participated, and pre- and post-intervention assessments were conducted to measure changes in MDRO prevention indicators. Following implementation of the SMART-based plan, there was a statistically significant improvement in staff knowledge (p < 0.05), with increased hand hygiene compliance (72.1–94.2%) and isolation gown use (60.2–87.6%). MDRO infection rates decreased from 8.0 to 5.1% post-intervention (p = 0.027). These findings demonstrate the effectiveness of the SMART-based infection control plan in reducing MDRO infection rates and improving staff compliance in the emergency ICU setting.

Efficacy and Safety of Single-dose Fosfomycin for... : Journal of Mid-life Health viewers conducted an electronic search on until December 2023 to search for randomized controlled trials that studied the efficacy and safety of fosfomycin for management of uncomplicated UTI in women or ASB in pregnant women and included those that fulfilled the eligibility criteria. The results showed that single-dose fosfomycin was comparable with other antibiotic agents in clinical resolution of uncomplicated UTI (odds ratio [OR]: 1.11, 95% confidence interval [CI] 0.88–1.41, I2 = 0%, P = 0.37, moderate certainty). Moreover, single-dose fosfomycin was equal to other antibiotics in the microbiological resolution of uncomplicated UTI (OR: 1.02, 95% CI: 0.83–1.25, I2 = 34%, P = 0.88, moderate certainty) and ASB among nonpregnant women (OR: 0.76, 95% CI: 0.45–1.28, I2 = 0%, P = 0.30, moderate certainty). The incidence of adverse events too was similar in patients receiving fosfomycin or other antibiotics (OR: 0.97, 95% CI: 0.77–1.23, I2 = 41%, P = 0.82, moderate certainty). Administering a single dose of fosfomycin is just as safe and effective as other antibiotics in treating uncomplicated UTIs in women. Prescribing a single dose of fosfomycin (3 g) as a regular practice may result in higher compliance rates and a reduced likelihood of antibiotic resistance....

NSQIP and NHSN: A Tale of Two Definitions Background: Infection Preventionists use National Healthcare Safety Network (NHSN) definitions to report required healthcare-acquired infection (HAI) measures. These measures can be publicly reported, and influence pay for performance metrics. The National Surgical Quality Improvement Program (NSQIP) also uses definitions to identify HAIs but is a quality program designed to drive outcome improvements. Extended Intravenous Bag Hangtimes and Risk of Central Line-Associated Bloodstream Infection in an Acute Care Hospital Background: Previously, the recommended maximum hang time for nonadditive intravenous (IV) bag solutions was 24 hours due to a cluster of contaminated IV fluids in the 1970s leading to bloodstream infection. However, in 2002 no recommendation was made, yet the norm of a 24-hour maximum hangtime remains widespread as limited data exist that demonstrate the safety of longer hangtimes. Because of IV fluid shortages, the hospital policy changed from a 24-hour maximum hangtime to 96-hours. To evaluate the safety of extended hangtimes, we used an ecological design to evaluate the relationship between the change in policy on maximum hangtime and central line-associated bloodstream infection (CLABSI) rate.
