I’m at the point where the best way to find amyloid in an echo is: if your amyloid sense is tingling off the plax, run the ekg thru @cardslab.bsky.social ai, if it’s over 20%, call it. Running strain and rwt isn’t worth the squeeze. Would love all 3 but our lab is stuck in 1999 #echosky #cardiosky

My chats with patients basically boil down to, waiting seems to increase stroke risk and you’re probably not going to stay asymptomatic for years anyway so might as well just pull the tavr trigger now. Maybe we need a doac v early tavr trial powered for cva

Oh ok. I don’t put on my critical thinking hat for non cardiology trials. Guess the grass isn’t that much greener

He’s probably mad about hospitalizations in the endpoint. Fair point. I’d like to see total hospitalizations. Concern that irrrversible fibrosis occurs while waiting and you wind up with worse chf after you eventually get the tavr. Don’t know why evolved trial didn’t look at 30 month ecv

What was wrong with early tavr?

Yah and if you use 0.4 eroa as your cut point (like my institution), there’s not too many patients that get so massively remodeled that you don’t label them as disproportionate

They opened the dnc with a land acknowledgement last August. Feels performative. it’s gonna come down to primaries in 27 if they go the Bernie aoc route or something less appealing. Until then. Don’t think they’ll have a central message

Ranked Choice Voting is getting a lot of attention but what is it?

It’s a simple ballot upgrade that gives voters more options and the freedom to vote their hopes instead of their fears.

Michigan deserves more voice and more choice. Join us.

Colbert Says CBS Pulled Senate Candidate Interview

Seems like a tempting loophole here. If you have to give equal time, just give 10 minutes to the QAnon shaman or some other whack job

Doesn’t seem there was ever a consensus on this from the teer trial meta analyses. I see a lot of chatter clinically on the lvef and lv volumes since the mitra-fr apparently had big low ef ventricles. They also had Eroa 0.31 on average. Way lower than anything I’ve ever referred

I would. Have had patients rejected when their ef is super low like <10%

Detecting Transthyretin Cardiac Amyloidosis With Artificial Intelligence This nonrandomized clinical trial reports the development and evaluation of an artificial intelligence–augmented screening program designed to detect transthyretin cardiac amyloidosis.

Point goes to the robots. 250 amyloid patients flying under the radar, picked up by mass ecg screening. Nnt for tafamidis is 7.5 so that’s 40 lives saved. πŸ‘ they set their screen threshold at 50%. Way too high a bar. Gonna miss most patients. I use 10%.

Would be more interesting to see prognosis of amyloid positive carpal tunnel biopsy. Cleveland data suggests very few of these get systemic amyloid in the near term

Patient: well my grandmother had high blood pressure

Classic mandrola. Always bitching about absolute treatment effects being too small. By his logic , every downhill skier would tie because winning by a few milliseconds isn’t big enough a win. Medicine is a game of inches. You take a rrr of 20% here, 15% there, eventually it all adds up

Hfref not hfpef in this trial but I feel like it’s worth bayers money. I only prescribe finerenone if the patients hit their $2000 copay max and I even prescribe Vericiguat. I don’t need all cause mortality reduction, I’d take a win ratio stacked with qol and biomarkers. I’m a cheap date

Recently the β€œMake Elections Great Again” bill was proposed in the House. Despite the name, it attacks voters, local elections, and Ranked Choice Voting by stripping authority from local election officials through sweeping federal overreach.
