You have to work until the age of 65 when you can access Medicare. You STILL have to buy supplemental insurance while on Medicare! Until then, you have find your own if you don't have it through work. I will lose my spouse's great insurance when they retire & have to find my own..

Before the ACA option (which I have not used yet but was always glad it was there as a choice), when I was self-insuring for years, I wouldn't seek treatment for things like migraines because having that on my record was grounds for denial of insurance coverage for that pre-existing condition.
