On the right lines…

It’s clonorchis sinensis, which commonly sit within the bile ducts in the liver, and are often seen to have smooth walled bodies and two gut branches.

Another liver fluke would be fasciola hepatica, but this would have spiny tegumens/a spiny outer wall.

In the UK, gentamicin cream is pretty much unheard of, and I have zero experience of using it as a topical cream. However, I’ve seen it being sold in pharmacies in other countries.
