Most of all, listen to clinicians who work in the NHS. We’ll tell you how to make things better - we stare it in the face every day.

Protect against claim culture that makes clinicians over-test out of fear. Educate people on, and instil the responsibility to, self-care. Stop target-setting, it makes care worse. Stop short-term initiatives. A year is not long enough to make and demonstrate any meaningful benefit on anything.

This is the case with the NHS. This is where the govt can intervene. Stop asking so much of the NHS. Be honest about the reality. Stop setting such high public expectations. Get NHSE to stop writing nonsense about early help for viral illness & aspirational vignettes on CT scans that aren’t needed.

The NHS is not broken.
The problem is not the NHS, it’s what it’s being asked to do.
If you asked Mo Farah to run 5 marathons in a day, and he couldn’t, you wouldn’t say he’s failing. You’d say the ask was unreasonable.

Serious shortage protocol.

Magic fixes for viral illness before Xmas must be subject to an SSP, surely?

I hope you bat that nonsense back in the way it deserves.

When I know appts are coming for pts in secondary care, maybe I’ll start writing and asking the rheum to β€˜kindly’ do the diabetic reviews as they are β€˜best placed’ and it’s more convenient for the pt. If they could forward on the results of the tests then we’d be grateful.

Sound ludicrous? It is.

I wonder how it would go down if, just as secondary care reject our referrals, I reject secondary care referrals for primary care appts?
You want me to do xyz? Sorry, you haven’t provided adequate clinical information. I won’t stoop to the level of the request coming on the wrong form.

Today I have had time to care. I always care, but to be actually able to translate that emotion, desire & passion into reality is a joy. Things need to change so doctors have time to care. It will be so much better for everyone.

That’s quite impressive. (And rather sad.)

The other place is really very angry, by comparison, isn’t it?

Not a podcast fan. Is there a transcription?

Bluesky definitely has some shortcomings cf X.
Lack of option to write longer posts and the silly long winded handle are two biggies. @bsky.app We’re all up for you but you don’t have it on the nose at the moment.

Amanda Pritchard β€˜β€˜welcoming’ the review into PA/AAs. Flaky at best.
We need assistants not associates. What they’re supposed to do, on a β€˜crash course’ qualification, could be done infinitely better by existing OTs/physios/nurses. Invest in staff with experience.

Finding people on here seems weirdly hard.

Absolutely!

Wes.
β€œHold tight” and wait for funding allocations. Our partner leaves on 26th December. β€œHold tight” is a loss of 8/34 sessions worth of GP time at our practice. 23.5% reduction in GP availability because we don’t know if we can afford to replace him.
This needs sorting NOW.

I’m lonely on here. A few followers down compared with the other place.
Please follow me and know you’ve made someone happy.

Anyone else find the .Bsky.social appendage unnecessarily untidy?

Anyone else dragging themselves away from the other place on principle, but sad at having to start over?
