So a little known fact is that music can sometimes influence what I draw, so as a result of my listening to synthwave, I have drawn Mocha in synthwave :3

The more you inflate it the harder you have to squeeze and push into it to get anything. But I will say taking a wand to it is definitely a fun experience, or placing a vibrator inside of it as well~

So as one that owns an irl nulge It does kind of work like option 1.
It's definitely more comfortable than a cage, has the benefit of not being rigid and doesn't cause chafing. Another benefit is that you can adjust how much touch you feel, the more inflation the less you feel when touched.~

So I guess in my opinion it would be a mixture of both. 1 being the default state, 2 when the one The nulge is attached to is getting put to use.

For example, state 2 is only active when obeying and following commands, activity being used.

This trains the wearer that it feels good to be used~
