Unless they are for profit, Universities and Hospitals file for H1B's which are cap exempt and don't through the lottery. From the way I read it non-profits might be ok for now. Small firms will be hit for sure, if this stands!

That's the way I read it. Cap exempt H1B petitions can be and are filed any time.

Hospitals, especially rural hospitals and Universities will be decimated if this goes through. They have a big time lobby as well.
I think the whole section about national interest could be for healthcare, research etc. But as you said we need to be prepared for all eventualities.
