Efficacy and safety of minocycline in patients with acute ischaemic stroke (EMPHASIS): a multicentre, double-blind, randomised controlled trial Minocycline therapy initiated within 72 h of acute ischaemic stroke provided a significant functional outcome benefit compared with placebo at 90 days, without safety concerns. Future studies are need...

This study exemplifies how drug repurposing research can still meaningfully benefit patients in vascular neurology. Such approaches offer a promising, cost-effective path to future drug discovery for those affected by stroke.

Striking gap: ~20x recruitment incentives in non-academic vs academic vascular neurology. Many choose academia for research, teaching & impact. Are we appropriately investing in what sets us apart?

Reflecting on Predatory Journals and the Pressures Shaping Modern Academia: Is the Publish‑or‑Perish Mindset Fueling Predatory Publishing? As conversations around predatory journals continue to surface, I often find myself asking a deeper set of questions: How did we get here? Who enabled the growth of this ecosystem? Who sustains it? An...

I’m curious to learn from your experiences and perspectives. How do you think we can collectively shift toward more meaningful research practices?

I've never used anonymous surveys for real criticism.

Feedback works best when it’s open and accountable. Leaders need to know who's speaking so they can reach out and discuss solutions rather than guessing. Let's champion openness for true improvement. #Leadership

#Neurology never stops amazing me. Had an amazing case of monocular quadrantanopia on initial exam that turned out to be ischemic stroke. Differential based on CT included Temporal Crescent syndrome findings, but I had never seen a case. Here is the link:

Received news that all three abstracts from my fellowship research at UTHouston were accepted for #ISC2026 (and one at #SVIN2025). Grateful to have an awesome team.

Looking forward to connecting with colleagues in New Orleans.

Med Myth reminder:

If a patient hasn’t used 75% of prescribed meds in 9 months, it’s not treatment failure or a side effect. Adding more meds won’t help. Investigate non-compliance reasons first.

They self-reported? Just listen.

Listening is essential to the art of medicine

The biggest advantage of having great juniors @VUMCneurology is that once you design a project, others are willing to take it to the finish line. After getting IRB approval, I barely contributed 10% to it. Here is full text:

Promoting a toxic person or a sycophant and expecting them to be a great leader is like planting weeds and hoping for roses. Leadership demands & thrives on integrity, not ego or flattery.

Every time I see a patient for a second opinion who was recommended to see a psychiatrist for neurological symptoms that clearly have an organic origin, I realize some doctors have a hard time saying, "I don't know".

Asymmetric dystonia, contraction, and hypertrophy are not psychological.

If a junior trainee wants to quit a specialty after working with your department, check:
1. Schedule
2. Leadership
3. Team dynamics
4. Selection process

100% chance you failed at one of the above.

2/ We must address functional overlay as a potential comorbidity in neurological symptoms. Educating patients that symptom exaggeration doesn't helpβ€”and can sometimes harmβ€”is crucial for their care and trust. Compassionate honesty >>> confusion.

As neurologists, we must be clear with patients: treatments like IVIG are unlikely to work for Functional Neurological Disorders. Offering ineffective options breeds mistrust. If you're unsure, saying 'I don't know' is far better than false hope.

Thank you for sharing your experience. I usually limit Maxeran to 40 mg due to plateaued effect above that dose and drowsiness in 1/5th of patient by that dose but would consider it in future if appropriate for individual case. Wondering if intranasal lidocaine is possible outpatient though
