I'm guessing the woman's thumb being like that with the plush mario is because they wanted more of the product shown, or the original stuffed toy she was holding obscured the thumb so they had to put a new one in.

That's bad photoshop for sure, if anything the babies' hands would be fucked up somewhere, also the usual tell are things in the background, like furniture having too many legs or nonsensical knobs, and plants melting into each other and it all looks fine.
