Somehow in order to care for Castiel, Dean needs to not care about Sam. Because you know, people can't have close interpersonal relationships to more than one person...

There are quite a few Destiels that are "Dean never really cared about Sam" and I mean not just romantically, but in general. And yeah, I don't think I would get along with that type of folks.
