Low Urine Cr concentration: simply very poor clearance in bad AKI. (But not in steady state; as even with GFR of 5 cc/min (72L/day), creatinine excretion remains the same)
Or high water intake.. low urine Cr of 20 mg/dl who is making 4 to 5L of urine a day is having adequate clearance (1000mg)

Person taking 150 to 200 meq of Na intake/day (to be in steady state) will have urine Na of 150 to 200 meq/L if they are making 1 liter of urine. (And Urine Na in 130s if 1.5L of urine per day). Number of possibilities with various Na &water intake. Kidney &its precision work!!

In a steady state, at a low GFR, Na reabsorption is often 80s to 90s%. & Urine Na is not uncommonly higher than serum Na.(Based on Na/water intake¬ accounting for diuretic that they often require
