The description of the investigation into this death reads: Adverse Event Description On 1/23/21 the patient had a single-car accident, slid off icy road into snowbank. She was seen in our ER, diagnosed w/ trauma and L4 compression fracture. She was transported to Hospital for further trauma workup. We believe she was treated and released. On 1/31/21 the patient had a headache but did not seek medical attention. In the morning of 2/1 she became unresponsive and was pronounced dead on the scene when EMS arrived. Autopsy showed a left temporal subdural hematoma.

The NIH and the government should be able to see where the money’s going.” “I’m really uncomfortable with this conversation, because you’re like, actually spreading rumors that you don’t know anything about. … Nature also is spreading rumors. Halt foreign collaborations, that’s not true.” “We’re working on the policy, Jocelyn. You shouldn’t be reporting rumors. I know there’s leaks all over here, but the leaks don’t actually reflect what’s happening. Don’t write about rumors. It actually makes the things that you and I care about worse. Like it spreads panic.” Later that day, NIH released a policy that halted future subawards to foreign scientists and said they will need to apply directly for money under a system still in development.

With NIH director Jayanta Bhattacharya dismissing my reporting as "false" and "spreading rumors" hours before releasing a policy that confirmed said "rumors," I feel compelled to respond.

Ah this might be a terminological issue - ‘clinical equipoise’ does not assume a 50/50 epistemological standard.

And certainly PFS or survival is not the only goal of treatment; patients should drive those decisions as much as docs and research should aim at a broad range of pt goals. In the vaccine case, I do see this as a bald-faced violation of pt rights against substandard intervention for no reason.

Right - that’s a more complex situation, similar issue come up with cost and adherence. Which, taking your point seriously, seems like a way of smuggling the patient perspective into a scientific question. I still think the issue is less equipoise and more what treatment endpoints matter…

Equipoise usually refers to the scientific community but not always (Alex London has written on this).

But there is no equipoise problem with evaluating previous research through RCTs. A study on lowering dosage to avoid SEs for example should use standard dosage as a control - just not a placebo.
