At the top of the page, there are three fields from left to right to write in spell's level, spell's name, and damage/type. Below are 4 check boxes (in this case, circles) labeled: Requires Sight, Ritual Spell, Concentration, and Components. Below those, on the left, 3 horizontal lines arranged vertically and labeled: Range, Targed/Area, and Target Saving Throw. In the middle, there are two fields labeled casting time(upper one) and duration (lower one). On the right, there are three horizontal lines arranged vertically, and the upper two are for materials, and the one at the bottom is labeled School. Below that, there is an ornate divider and college-ruled lines to write down the text of the spell.

Page for prepared and known spells. At the top, there are four boxes in a line with labels Spell Ability Mod, Spell Attack Bonus, Spell Save DC, and Constitution Mod. Below them is a header that says "Prepared Spells." Below the header, there is a table to fill in seven cantrips. There are 6 columns: Cantrip Name, Casting Time, Range, Duration, Effect, and VSM (vocal, somatic, material) circles to fill out. Below the cantrip table, there are four fillable segments to write your prepared spells in. Each segment contains an ornate circle to write in the level of the spell, two circles to fill in with labels Conc. and Rit., and six labels to write down the spell's name, casting time, range, duration, effect, and spell components.

At the top of the page, on the left, there are fillable circles to keep track of your spells. There are 9 columns with spell level labels, the first row has nine star-shaped fields to write in how many spell slots per level you currently have available, and below are fillable circles for spell slots. On the right of the spell slot table, there are two fields vertically placed. The upper field is for you to write in the name of your spell list preset. (spells for combat, spells for downtime, spells for travel etc). The lower field is to write in the number of prepared spells you currently have available. Below that, there are eight fillable segments to write your prepared spells in. Each segment contains an ornate circle to write in the level of the spell, two circles to fill in with labels Conc. and Rit., and six labels to write down the spell's name, casting time, range, duration, effect, and spell components.
