Selfie in front of Queenβs Medical Centre main entrance, with the facade lit up by green and purple floodlights and with Christmas illuminations along the front canopy. PTS ambulances are jostling for space along the front of the building behind me.
Heading Londonwards from visiting QMC in Nottingham to talk to obstetric doctors there (and management) about strikes and derogations. Clearly a department facing huge challenges, with doctors run ragged to make up for deep-rooted problems. Thank you for the frank discussions. @mbdoc.bsky.social

Water plant that caused Tunbridge Wells supply failure was given risk notice in 2024
Watchdog investigating South East Water contamination issue had previously warned of potential health danger
Failures at South East Water owned water treatment centre led to tens of thousands of people in Tunbridge Wells being cut off from the supply.
In 2024, regulator warned SEW over bacteria and pesticide contamination risks ... nothing happened.

Confirmed. Resident Doctorsβ strike in England, 7:00am on 17 December to 06:59am on 22 December.
After the last round of strike action, resident doctors in England gave Wes Streeting a window to deliver sufficient progress towards a viable deal on jobs and pay.
That hasnβt happened.
Residents have been given no choice but to return to the picket lines.

Doctors should not have to strike to be heard.
It's not too late for Wes Streeting to return to the negotiating table and avoid this industrial action.
We remain committed to negotiating a solution on jobs and pay for resident doctors in England.

Graph from the 2025 GMC workforce report, showing 10 year trends in professional groups. The number of locally-employed doctors, on non-standard, non-national and usually non-permanent contracts has increased fourfold in a decade. LEDs now outnumber SAS doctors, on nationally-negotiated contracts, roughly 3.5:1.

Today's BMJ investigation exposes the widespread exploitation of locally employed doctors, who face dire terms and conditions and lack clear development opportunities.
We're clear the Government must urgently address this situation by mandating fair, standardised contracts.

Reading the report really took me back to those days all doctors and NHS workers will remember; knowing it was coming, knowing it was going to be bad.
The parts about the opportunities squandered - not just early on but repeatedly - to reduce the impact are really hard to read.

Relevant safeguards could includeβ¦
Engaging with IT software providers to find IT solutions that work for patients and practitioners.
Flexibilities in allowing practices to suspend online triage when they become overwhelmed on a working day and safe care is threatened.
Increasing practice resources and staffing capacity to manage online requests and queries beyond the end of the currently contracted core hours of 8amβ6.30pm.
We surveyed GPs across England to find out how these changes are impacting practices.
More than 1 in 5 practices in England responded, with 55% reporting adverse impacts on patient care from changes to online access.
These risks could be mitigated if the Government implemented relevant safeguards.

negative impact on patient care due to online access changes

Yet, our concerns have been continually dismissed.
