That's my set up as well. Make sure you have terminal sounds set to on in VS Code settings. It didn't work for me until I did that.

claude config set --global preferredNotifChannel terminal_bell

For macOS users: Don’t forget to enable notification permissions in System Settings → Notifications → [Your Terminal App].

If using VS Code you need to ensure that terminal sounds are set to on.
