NAP8 will be collecting data around complications of regional anaesthesia next year covering all UK for 12 months. Hereโs hoping serious complications are really unusual but sadly we know they happen @rcoa-cri.bsky.social #AnSky

More seriously, perhaps AI and smarter technology might be of benefit in reducing the risk of fatal local anaesthetic toxicity. This reporting database study from the US highlights this rare but potentially catastrophic complication still occurs. #AnSky bsky.app/profile/bjaj...

Donut chart showing relative numbers of provider types in deaths reported due to local anaesthesia from the BJA paper by Fettiplace and colleagues. Anaesthesia: 23; self: 18; surgeon: 6; dentist: 4; pulmonologist: 3; ICU: 6; ward nursing: 3; ward cardiology: 1.
โAnaesthesia was the most frequent provider (36% of cases) but also with delivery by proceduralists and on wards (Fig. 5b).โ Without knowing the context of frequency of LA use (especially with potential for higher doses) difficult to ascribe any concept of relative risk #AnSky

There is a U-shaped relationship between BMI and the risk of postoperative loss of independent living.
Patients with mild obesity experienced a lower risk of losing the ability to live independently after surgery.

Shocking reality: 1 in 4 older hospital patients develop #delirium, yet most families have never heard the word.
We need to do much better in improving public awareness of delirium.

Letโs try a little #MedSky game. When I finished med school:
-gastric lavage had been scrapped for ODs
- Doxapram was still used for T2 resp failure in COPD
- co-danthrusate was widely used,
- Quinolones were magic and only consultants were allowed to prescribe them.
What year did I qualify?

Love this, when I finished med school
- cefotaxime was the latest antibiotic
- we had a new look BNF
- we wore white lab coats with nice big pockets
- H. Pylori was starting to be linked to ulcers (yes I did gastro house job)
- vagotomy with Pyloroplasty was a common treatment for GI bleed

Thereโs lots we donโt know about delirium. To help researchers ask the right questions, not necessarily the ones we think of, it is really valuable to have the input of clinicians, patients, carers and others. So click the link and tell us what you think. #AnSky www.surveymonkey.com/r/WTHNBZK

Our UK SNAP3 cohort of older surgical patients reported delirium occurred in around 1 in 16 overall and 1 in 5 for those living with frailty. Itโs not just those with hip fracture either. Older age, emergency surgery have higher risks #AnSky #SurgSky www.bjanaesthesia.org/article/S000...

We told my Dad frequently that he had delirium & that it was because he was unwell, 2 years ago.
I think it did help, & he volunteers to us regularly now that heโs had strange experiences. Heโs got definite dementia now, but Iโm happy that he isnโt inhibited in telling us when weird stuff happens.
