It would have been interesting to see plans for implementation and risk assessment. I've seen one version from a local authority that was promoting one of these interventions in all its schools as a universal intervention for all pupils. Seems rather indiscriminate...

...My hunch for the tiles is similar, something under the tiles in the middle is conducting thermal energy better than the corners and so they are melting more quickly from the warm ground underground.
