Afraid to say I suspect you are missing the point - I've seen what Nicole has said previously on the subject and she has been consistent that a desire - regardless of access - means transsexual under this model. I suppose closeted gay people are also missed under our model of homosexuality,

Frankly it sounds like everyone involved in this conversation is recognizing that, so I feel like there is some confusion here. Your friend would not be considered cissexual under this framework, in the same way I would not consider a celibate gay man to be straight

Many gay people can live out their entire life without a fulfilling relationship that reflects their actual desires - and have - but we still understand them to be gay because we recognize the underlying desire. Not all "needs" when unfulfilled lead to death.

I don't even see why this is hard to get. We already have a common cultural understanding that being gay is defined by an endorsed "want," but also understand that this want is a reflection of an internal difference that is in-born. Why can't the "want" to change sex reflect an internal difference?
