Maybe that would incentivize them to stop making patients and physicians jump through these hoops to get essential care

I've got two big problems with this.
The first is that it's dishonest.
Hospitals know how much revenue physicians bring in. Here, e.g., are figures from a 2016 survey.
Rather than "losing $300k", the average PCP/specialist *generated* revenue of $1.4 million and $1.6 million, respectively.
