The palliative care evidence dilemma
Newer drug
Limited evidence therefore not widely adopted
Or good evidence yet not widely adopted
Established drugs
Mostly very limited evidence or off label use yet form the foundation of symptom control - remains mostly unchallenged
A new "Try the 4AT" calculator was added to the main 4AT website just over a week ago.
Since then >1000 visits with >2min average per visit - great to see so many people trying out the 4AT.

Promotion of diuresis through osmotic glycosuria? Fascinating drugs, came into fashion when I was doing some diabetic clinic work. Main issue was UTI and candida which seemed quite common

How many other drugs used in palliative care have this level of evidence?

2/2 I routinely discuss this with patients now, if effective then discontinuing the drug will lead right back to the original problem. Same goes for stimulants such as bisacodyl.
As always prevention is better than treating advanced constipation.
I Donβt wait weeks to try a PAMORA
PAMORA observation 1/2
When using Naloxegol or Naldemedine for opioid related constipation the first couple of doses are likely to result in abdominal cramping and loose stool as the constipation resolves. These side effects usually pass once hard stool and overflow subside
Interesting report on people who frequently attend A&E via ambulance, with palliative patients making up a large proportion of the cohort.
