During the worst of the pandemic I'd make my way home after a shift on COVID ICU listening to songs on a playlist I kept.
Just listened to a few songs on this playlist this evening and suddenly felt a surge of emotion wash over me.

Keir Starmer promises migration drop as he unveils plans for 'tighter' visa rules
Plans to ban the recruitment of care workers from overseas are among efforts to curb near record net migration.
Thanks for highlighting Jon. There's much work to be done on this, but this is all in whilst governments around the world are turning their backs on immigration. Xenophobia, racism and ethnic diversity are all linked... This problem extends beyond anaesthesia.

This is fascism. The censorship of scientific literature has begun. A new dark age is upon us.

Look after yourselves everyone. Apply the oxygen mask to yourself first. If you need to step away for a bit, do.

I know we've all tried to move on. But I'm not sure we ever will truly move on. We probably just put it to one side for a bit. Until another moment reminds us of our past.

This was definitely true during COVID, when in the UK loads of us saw in hospital deaths on a completely unprecedented scale.

But for medics this could sometimes be daily occurrences, and there's very little opportunity to process or vent off the emotions that might build up.

This time he punctuated the humour with very dark truths that left me reflecting on my way home... In medicine we see some really grim scenes, and deal with some terrible situations. Often other people only encounter these situations infrequently in their own lives. Like death or violent trauma.

I had the privilege of seeing Adam Kay's This is Going to Hurt show last night when he came to Singapore. Saw him play in the UK when we was part of Amateur Transplants, but back then it was all just funny songs and comedic gags.

And truly I know people who didn't. So many people burned out and either left healthcare or moved on to other specialties, or in some sad instances left our sides completely and permanently.
I hope we never need to do it again. But we should never forget that period either.

We had to have 4 ventilated patients looked after by 1 fully trained ICU nurse, supported by some ward nurses who were redeployed to ICU, who never really had experience working with ventilators or rows of infusion pumps. I dunno how we survived it.

I also remember the first patient I had to intubate for COVID respiratory failure, and the times spent on ICU proning and unproning patients. There was one particularly bad time when we had to immediately stand up an emergency spillover ICU, converting surgical recovery into an ad hoc COVID ICU.

Me wearing PPE outside of COVID ICU.
Can't believe it's been 5 years since we first went into COVID lockdown in the UK. It was such a crazy time when I look back. I remember ordering a couple of reusable FFP3 respirators online a few weeks prior in case we were going to run out of disposable ones at work.

I was scrolling through another social media platform reading the comments. There were just a bunch of people saying pointless stuff like "Nicely articulated." And "Very informative!" Completely AI generated emptiness. Compute cycles are being wasted such vacuous drivel. Meanwhile the ice melts.

It is no longer safe to go to the USA for any reason at all. I shall not be going there for anything. Not for a conference, not for work, not for a holiday, not to do anything there. It holds no purpose for me to visit.
