As the African proverb puts it, βIf you think youβre too small to make a difference, you havenβt spent the night with a mosquito.β
One person can stop a great injustice
One person can be a voice for truth
One personβs kindness can save a life
Each person matters

My six favourite European Mr Men character names...
6. Mr Muddle in Spain = Don Confuso
5. Mr Topsy-Turvy in Germany = Herr Kuddelmuddel
4. Mr Bounce in France = Monsieur Bing
3. Mr Dizzy in Portugal = Senhor Bobo
2. Mr Greedy in Sweden = Gubben Glufs Glufs
1. Mr Bump in Norway = Herr Dumpidump
A song to remind me of the most useful phrases I have learnt from Italian Duolingo. (Sound up)

17.11.2024 22:57 β π 1 π 0 π¬ 0 π 0At one of my first meetings as a council member of the Royal College of Surgeons of England, we approved a report called Access All Ages. It encouraged less ageist thinking and bias among healthcare staff that might lead to them denying older people surgery. But sometimes an operation isn't the best option. Among patients who have surgery, 14% express regret and 15% experience complications, which are at least four times as likely if they're frail or physically inactive. The Centre for Perioperative Care has published information on the importance of exercise before surgery, but that alone may not be enough. We need shared decision making, including asking patients what matters to them. The public should be primed to ask about BRAN-the benefits, risks, and alternatives to surgery and the likely result from doing nothing. A slew of data supports this approach, especially from the Perioperative Care of Older People Undergoing Surgery (POPS) initiative. After discussion with a geriatric medicine team completing a comprehensive assessment and using shared decision making with a POPS approach, 14.8% of patients decided against surgery. Most of these then had their health improved through other means: adjusted medication, lifestyle advice, or other interventions.
This is a very helpful piece in the @bmj.com from @scarlettmcnally.bsky.social on improving shared decision making in medicine (here specifically, surgery).
When we talk about waiting list numbers, Iβd be keen to know whether everyone on them has had a thorough review as described.

Absolutely this π.
One of the best explanations about this describes how 'nice culture' can be as harmful as 'nasty culture'- and much harder to call out because it casts the dissenting person as 'not nice'. But 'nice culture' stifles cognitive diversity.

A photo of me speaking at a conference. Behind me is a slide that says reframing our language with the words wellbeing and resilience
Let's be clear,
Wellbeing in healthcare is not fluffy stuff.
It's about workload, staffing, systems & culture.
It's about valuing our own needs,
modelling humanity & vulnerability,
Creating spaces where people feel able to be authentic & ask for help.
Wellbeing must be a priority.

Continuing with the Plan A Block re-cap, we have the Axillary Brachial Plexus Block
This is my favourite block to teach novice blockers
Great for hand and forearm pain, surgery & fracture manipulation
