His sternum is cracked open and his chest cavity is empty. The moment depicted in this piece is his first "breath" as a Lich: he undergone the gruesome trasformation that left him 4 days in a coma and for that time his body was laid on a stone altar, covered by a tapestry from his Enclave, guarded by two Will-o'-wisps. Now he finally woke up,undead, undying... and no longer needing to breathe at all.
