I can't give you a definite answer for how it is handled. I'm going to assume 2d with layers since the feature was added much later in development. Luckily, CDDA is open source so you can take a direct look at the mechanics if you're interested.

CDDA does this, you could get onto a rooftop and take your time plinking at mutants in the streets. It did require a toggle for cross z level vision but that might be different now.
