I'd say that was a failure of role segregation/ supervision. That doesn't happen at my hospital.

Yes they can suggest, but it should be a senior doctor that makes a diagnosis.

They provide a junior doctor role - writing notes, ordering and chasing tests, speaking with relatives. There aren't enough junior doctors to support the current consultant numbers.
