I was a nurse assistant at a hospital and mostly dealt with older adults. I would draw blood for blood tests and the hand is a very easy place. Also very easy access for IVs. Again reckless speculation but the pieces add up.

I would like to recklessly speculate that he is getting an IV treatment or something. The bruise along with the admitted high aspirin dose could create a consistent bruise on that part of the hand. It's a common place for an IV or blood draw, especially while wearing long sleeves/suit.
