i am no expert on any of these, nor do i have any education in any of these areas, but as someone who takes a greater-than-average interest in them, i can try to give a basic reasoning as to why these are factors.

1. Rigid patriarchy/hierarchy - Men must be in control. Men must be in charge. ....

there are many many (many) reasons why it won't happen in mainstream media (or even mainstream-adjacent like GDT's amazing works), but it boils down to the classics of "wait why isn't this in modern media" - rigid patriarchy/hierarchy, misogyny, and the ever-present 'performative male' zeitgeist.
