THE GREAT DEBATE: Blood-Based Testing:
๐นโฌ๏ธ in CRC incidence/mortality NOT as dramatic as for FIT & colonoscopy
๐นHigh sensitivity for any stage CRC, though notably lower for stage 1 CRC and low for advanced precancerous lesions
๐นWould take a VERY โฌ๏ธ participation to be = FIT
Iโll be watching!!

It is our responsibility as GI doctors to directly inform patients who are found to have advanced adenomas:
โ
Future surveillance should occur at no > 5y intervals
โ
Children? If so, should be screened starting at age 40!
I always โ๏ธ patients to tell them verbally!๐ฃ๏ธ

04.04.2025 22:47 โ ๐ 0 ๐ 0 ๐ฌ 0 ๐ 0Role of FIT testing in Lynch syndrome?
๐น~70% of colonoscopies in Lynch do not reveal neoplasia
๐นThreshold of 4.1ยตg Hgb/g feces maximizes detection of CRC, adv adenomas (89% sensitivity, 99%NPV, 94% specificity)
๐นConsider FIT in Lynch pts at low risk for CRC to extend colo interval?

Thank you for sharing this! Absolutely agree that we need to do better in not remaining siloed in our specialties. Multiple cancers over multiple generations = red flag!

Q5: What are some of the latest advancements in the treatment of hereditary colorectal cancer syndromes?
#Gastrosky
9/
VUS: genetic alteration w/ unknown disease risk
๐งฌ Common, found in 30-50% of patients undergoing multigene panel testing
๐งฌ >90% of VUS are re-classified as benign over โฐ
โ ๏ธ Do ๐ซ recommend changing medical management based on VUS alone!

Q4: What are the implications of identifying a โvariant of uncertain significanceโ on genetic testing๐งฌ?
#Gastrosky
7/
NCCN guidelines excellent but complex ๐๐ฝ

Broadly speaking, think about referring if:
๐ Early onset cancer age < 50
๐ Multiple cancers or >10 polyps
๐ Multiple cancers on same side of family
Note: absence of family hx does not ๐ซ a hereditary syndrome!
6/
Q3: In what clinical scenarios should gastroenterology providers think about referring patients for genetic testing๐งฌ?
#Gastrosky
5/
๐Simple 3Q screening tool (Kastrinos et al. 2009) is feasible!
1. FDR < 50 with CRC, CA of uterus, ovary, stomach, small intestine, urinary tract, bile duct, pancreas, brain?
2. CRC or polyps at age < 50?
3. 3+ relatives w/ CRC?
โ
High sensitivity

Q2: What are the key elements of a family history that should be collected to assess the risk of hereditary colorectal cancer?
#Gastrosky
3/
Hereditary CRC makes up:
๐น10% of all CRC
๐น16% of all CRC in age < 50
Most common: Lynch syndrome
Caused by:
๐งฌMMR gene mutations including MLH1, MSH2, MSH6, PMS2
or
๐งฌEPCAM mutation which sits upstream of MSH2)
Prevalence: 1/279 individuals in general population๐ฅ
