I have not read piece as not willing to pay for it but I suspect it is job availability and an opportunity to experience other parts of the world for a year or so that attracts not (just) the pay. Make sure U.K. med grads are prioritised for training before paying even more.

It’s pretty bloody obvious what it means. They didn’t inherit it or were otherwise given it they earned it by hard graft, taking risks, good judgment and skills.
