As Seth mention, if you draw a line in the sand on something, they are willing to suck it up and get it done for you
So if they need to punch out early for a kid's game, school thing, etc, I'm going to be flexible
If they are not feeling well and need to go, we watch out for them

All of us get busy with multiple patients, calls, etc
A staff who is comfortable helping you not screw up is a huge help, being the traffic cop to tell you to pick up this room first as it will be nice for you to not have two complex cases in a row ... they watch out for you

Humana is now using an AI system to process claims and it is A MESS.
While a computer algorithm was used in the past, it for the most part worked

The nuance here is MA plans must spend 85% of premiums on "clinical care"
PBM fees & rebates get counted in the 15% administrative cost allowed, the amount paid to the pharmacy is considered clinical care
Ever wonder why these plans are now pushing their own mail order pharmacy plans? This is why.

The top 5 plans own their PBM, creating additional revenue by designing formularies which have higher cost drugs (with higher cost share) for which they receive (and keep) rebates
If you want an ethics cost in this space, look at the Pharmacy Benefits Manager (PBM) system
Originally, PBMs "uncomplicated" plan formularies, brought cost down
The top 5 MA plan entities now own a PBM & it is now a revenue stream, at a cost to patients, with rebates to plans for higher cost meds
Mayo discontinued participation in Medicare Advantage (MA) plans from both United & Humana
I see this as a MA problem, not a Mayo problem
MA plans were originally to save the government money, paying the plans about 93 cents for every dollar of cost under traditional Medicare, its now $1.20
