The panel may recommend it, but you can definitely give cefazolin as surgical prophylaxis for patients with anaphylaxis to other beta-lactams.
Many hospital systems in the US do it and have not seen increased rates of anaphylaxis

Always worth reminding people that cephalexin and cefazolin are very much not the same in terms of R groups.
I often see people assume that since someone tolerated cefazolin the patient won’t develop a delayed rash to cephalexin

Obviously I have concerns from a stewardship perspective, but if we are giving 5 days of ertapenem to patients with a disease with 20+% mortality, it may not be that big of a deal.
