I did one a few years back that essentially turned into a cake batter during second dough formation. I'm far from a master at the LM prep, and have been just using very fresh (refreshed many times over many days), very active, sourdough starter (instead of a cultivated LM) with good results.

Damn, I'm sorry. I've been there, and I know how frustrating it can be, but those will still taste great. This very thing has happened to me, and I have narrowed it down to likely being a Lievito Madre issue. The oversimplification is excessive acid in the dough can break down the gluten structure.
