❓ Q: My patient with febrile neutropenia (FN*) on IV cefepime is hemodynamically stable but continues to have fevers. Do we need to switch empiric antibiotics to IV meropenem?

Not only is it Black History Month. It’s also its 100th anniversary!!

To commemorate its centennial, I wrote a list of 10 Black films, one from each of the last 10 decades, to stream.

this is exactly right. "popularism" in practice amounts to an abdication of political leadership!

The press has done a lot over the years to manufacture presidential gravitas and prestige, going above their expected duties as state stenographers.

But I don't think any other president has had so much free PR and image management coordinated across so many media outlets, not even close

Alex Pretti was a colleague at the VA. We hired him to recruit for our trial. He became an ICU nurse- I lover working with him. He was a good kind person who lived to help and these fuckers executed him.

White. Hot. Rage.

Very hectic but rewarding weekend of ID consults. Not a sustainable volume, but did feel like I did good work.

if you were in college during your first β€œNo Blood For Oil” protest then now is a good time to discuss with your doctor which age-appropriate cancer screenings are right for you.

Thought I'd repost a relevant skeetorial from last year!

I often get asked about renal dosing adjustments of Abx in critically ill pts in the ED, so I'd thought I'd share my thought process & perspective. Caveat: only in ED! Lets dive in! πŸ‘‡

ID once again dragging ourselves across the finish lineβ€”which is a crying shame because I love my job and I love treating people with infections. IM residents, ask me about my work and I’d be happy to sing its praises!

There’s also an emerging body of evidence showing that reliance on AI eventually leads to de-skilling of humans, and we need a better sense of the long-term consequences of that (e.g., are we comfortable with a future where radiologists are just worse at interpreting mammograms?).

30.11.2025 09:48 β€” πŸ‘ 4    πŸ” 0    πŸ’¬ 0    πŸ“Œ 0

This is the challenge of any cancer screening program: knowing whether detecting cancer before it causes symptoms saves lives is challenging! We’ve gone back and forth on this for prostate cancer and breast cancer for decades. AI *may* help, but I don’t think we have good data for that.

30.11.2025 09:48 β€” πŸ‘ 3    πŸ” 0    πŸ’¬ 1    πŸ“Œ 0

There’s a more sober approach to this that is being missed: we don’t know if these AI-driven analyses would save lives or outweigh harms of overdiagnosis. AI could be better at screening for cancer *and* still not be useful for screening (in the way we do breast or colon cancer screening).

Lots of people sharing this clip because it's funny and cathartic but I also genuinely believe it's important.

We have to win the narrative battle between "I live in a corrupt society and that's just how it is" and "I live in a corrupt society and we are going to defeat and humiliate these ghouls."

Louisiana restricts access to abortion pills by classifying them as a controlled substance A bill that would restrict access to abortion pills has been signed into law in Louisiana. The law reclassifies the drugs as a controlled substance, grouping them with Xanax and Valium. Possession wit...

Scheduling mifepristone largely invalidates the point of the schedule.

Ostensibly, the schedule is meant to monitor drugs *which have the ability to form physical dependence*.

This step makes the schedule a list of drugs that lawmakers dislike for feelings reasons.
