Would be a small led light in mouth, easy to close lips around. What makes me think he has one is light in his mouth and light on the ground go on/off together a couple times in vid. If that was reflected, there would be one cast at all times from bag reflectors which have to be brighter than teeth.

If you don't look at his face, just the ground around him at the moments there is a bright spot in his mouth, you can see it's casting light on the ground, just a few blips for a split second. I get what you mean about hot spots in the video from teeth etc. but they can't cast light like that.

Honestly, labeling is THE thing that makes most maps and the subtleties involved are probably more than AI can do correctly 100% of the time. At least for the foreseeable future.
