some state medical schools offer lower tuition in exchange for X years of practice in the state. There's also pretty good evidence to suggest that people are more likely to stick around in these places if they do residency there

we're already lowering practitioner training standards (insofar as we are moving from GPs to PAs and loosely-accredited NPs) but they don't want to live in rural areas either!
